Medical Insurance
Employees have the choice of self insured health plans listed below. SVMC pays 100% of employee premium amount for the base plan. The employee pays any balance for higher plan or for dependent coverage through payroll deduction. Employees are eligible the first day of the month following completion of the 90-day Introductory Period.
Base Plan
$200 deductible; $20 office visit co-pay then paid at 80%; outside network benefits.
Buy Up Plan
$200 deductible; $20 office visit co-pay then paid at 100% after deductible; no out of network benefits
Both plans have reduced co-pays for office visits and
prescriptions when seeking services at SVMC.
Vision
One eye examination covered through health plan every 12 months.
$100 towards eyeglasses at SVMC Eye Care Services
Section 125 Flexible Spending Account Plan
Employees may elect pre-tax withholdings for dependent care (adult or child care) or health care expenses including dental, vision, hearing, prescriptions and over-the-counter medications.

SVMC compensates employees with pay for six legal holidays per calendar year:

Vacation time is accrued according to the following schedule for full time and pro-rated for part time.
Years 1-2... 96 hours
Years 3-4... 136 hours
Years 5 and above... 176 hours
Sick Time
80 hours of sick pay is accrued annually for full time and pro-rated for part time employees.
Long-Term Disability Insurance (must be minimum .75 FTE)
Effective 90 days from the date of disability; reimburses 60% of monthly earnings at the time of disability.
Life Insurance (must be minimum .75 FTE)
$15,000 coverage.

Confidential professional counseling services available through First Choice Health without cost to employees and immediate family members to prevent or resolve life problems and/or issues.
